In order to make your trip with IZE enjoyable, we've put together all the forms and handbooks travelers will need for their trip to Belize.

Books are in Adobe PDF format and can be opened with Adobe Reader, a free download.

IMPORTANT NEW BELIZE LAW - June 2010:
All Children 16 years or younger traveling without a parent/legal guardian must have a notarized letter of permission from his or her parent/legal guardian. Please call or email with any questions or concerns regarding this new law before your travel date.

Participant Forms
These forms must be filled out and signed by all IZE guests (and/or parent or guardian if under age 18) and faxed or mailed back to our USA office one month before your trip.

PADI Scuba Diving Forms
These forms need to be filled out and signed by all guests (and/or parent or guardian if under age 18) and brought with you to South Water Caye in order to participate in diving activities.

    •  PADI Medical Statement (RSTC)
    •  PADI Liability Release

Students and guests that wish to participate in PADI Discover Scuba classes should also fill out and sign (and/or parent or guardian if under age 18) the following form and bring it with you to South Water Caye in order to participate :

    •  PADI Discover Scuba Diving Statement
